PLEASE READ CAREFULLY: Medication
is normally given at 8:00 A.M., 12:00 P.M., 4:00 P.M. and 8:00 P.M. Each
camper must have the Medication Administration Record Form (MAR) completed prior
to scheduled camp session. Bring the MAR form with you at time of check-in and
make sure the MAR form is given to health care staff. Each
camper must bring enough medicine to last the entire session. If the supply is
exhausted, the prescription will be refilled and the parent or guardian will be
billed. Medicine MUST be provided in a LABELED PRESCRIPTION BOTTLE or BUBBLE-PACK
and include one additional days supply. Please keep any remainder at home. If
the camper is allergic to bee stings, an epipen must be sent along with the camper.
Campers with diabetes must bring all equipment needed (test kits/syringes) or
a fee will be charged for each test at a cost of $1.00. -
Each kind of medicine must be in its own original container and each kind of medicine
must be labeled with the name of the medication, campers first and last name and
directions for administration by physician. We cannot dispense medication otherwise.
- Do not pack medicine in camper's
suitcase. Please carry it separately so it can be given to the health care staff
when checking in.
- No medication
other than prescribed by the Camp Physician or the camper's physician will be
given to any camper.
- Campers must
have a medical exam by a licensed physician within twelve (12) months prior to
attending camp. School or Special Olympics exams will be accepted.

PARENTS/GUARDIANS WILL BE NOTIFIED IMMEDIATELY WHEN A CAMPER IS SICK OR TAKEN
TO THE EMERGENCY ROOM FOR AN ACCIDENT OR ILLNESS.
